Are you a skilled and compassionate Nurse Practitioner (PMHNP/APRN) or Physician Assistant with a passion for working with adults facing both mental health and substance use challenges? At CHR’s Pathways Program, you’ll join a multidisciplinary team that works every day to reduce harm, restore hope, and help individuals reclaim their lives.

Pathways is an accredited opioid treatment program (OTP) specializing in methadone-based MAT. We offer comprehensive residential care—including medication management, counseling, psychoeducation, recovery coaching, and individualized treatment planning—in a deeply supportive and mission-driven setting.

What You’ll Do:

​

  • Conduct psychiatric and medical evaluations

  • Initiate and manage MAT (including methadone and buprenorphine)

  • Diagnose and treat common acute health conditions (e.g., infections, wounds, respiratory illnesses)

  • Perform annual physicals and health assessments

  • Manage mental health medications and assess post-acute withdrawal symptoms

  • Educate patients on harm reduction, health maintenance, and wellness

  • Collaborate with your team to develop individualized treatment plans

  • Guide and oversee RN staff as they carry out medical orders

  • Maintain accurate and timely clinical documentation

  • Participate in team meetings, referrals, and coordinated care planning

 

Who You Are:

​

  • Graduate of an accredited PMHNP, APRN, or PA program

  • Licensed in the State of CT as a PMHNP, APRN or PA with an active DEA license, or ability to obtain

  • 3 years of post-graduate experience working with dually diagnosed adults preferred

  • Committed to patient-centered, trauma-informed, and recovery-focused care

  • Comfortable working in a dynamic, team-based environment with a diverse client population

  • MAT experience strongly preferred (training available for the right candidate)
