Are you a skilled and compassionate Nurse Practitioner (PMHNP/APRN) or Physician Assistant with a passion for working with adults facing both mental health and substance use challenges? Pathways is an accredited opioid treatment program (OTP) specializing in methadone-based MAT. We offer comprehensive residential care—including medication management, counseling, psychoeducation, recovery coaching, and individualized treatment planning—in a deeply supportive and mission-driven setting.

What You’ll Do:
​

Conduct psychiatric and medical evaluations
-
Initiate and manage MAT (including methadone and buprenorphine)
-
Diagnose and treat common acute health conditions (e.g., infections, wounds, respiratory illnesses)
-
Perform annual physicals and health assessments
-
Manage mental health medications and assess post-acute withdrawal symptoms
-
Educate patients on harm reduction, health maintenance, and wellness
-
Collaborate with your team to develop individualized treatment plans
-
Guide and oversee RN staff as they carry out medical orders
-
Maintain accurate and timely clinical documentation
-
Participate in team meetings, referrals, and coordinated care planning
Who You Are:

Graduate of an accredited PMHNP, APRN, or PA program
-
Licensed in the State of CT as a PMHNP, APRN or PA with an active DEA license, or ability to obtain
-
3 years of post-graduate experience working with dually diagnosed adults preferred
-
Committed to patient-centered, trauma-informed, and recovery-focused care
-
Comfortable working in a dynamic, team-based environment with a diverse client population
-
MAT experience strongly preferred (training available for the right candidate)
