At CHR, Community Outreach Therapists work closely with a small, manageable caseload of children and adolescents (ages 3–18) providing community-based therapy, case coordination, and support services designed to prevent hospitalization and promote emotional wellness to families and children ages 3 to 18.
The goal: prevent hospitalizations, promote stability at home and in the community, and support stronger family relationships.

What You’ll Do:
Provide therapeutic services (individual, family, and group therapy) to children and families.
Complete comprehensive, strength-based assessments and treatment plans.
Maintain collaborative relationships with families, schools, and community partners.
Document progress, discharge planning, and outcomes accurately and timely.
Deliver care in-office, in the community, and in clients’ homes.
Participate in team meetings, supervision, and agency events.
Support colleagues with case coverage when needed.
Transport clients as needed.
Who You Are:
Master’s degree in a mental health field (LCSW, LMFT, LPC or eligible).
Experienced or newly licensed clinicians encouraged to apply – we support your growth!
Skilled in providing therapeutic support to youth with emotional/behavioral challenges.
Passionate about helping families strengthen relationships and build resilience.
Motivated to make a meaningful difference in the community.
