At CHR, Community Outreach Therapists work closely with a small, manageable caseload of children and adolescents (ages 3–18) providing community-based therapy, case coordination, and support services designed to prevent hospitalization and promote emotional wellness to families and children ages 3 to 18.

The goal: prevent hospitalizations, promote stability at home and in the community, and support stronger family relationships.

What You’ll Do:

​

  • Provide therapeutic services (individual, family, and group therapy) to children and families.

  • Complete comprehensive, strength-based assessments and treatment plans.

  • Maintain collaborative relationships with families, schools, and community partners.

  • Document progress, discharge planning, and outcomes accurately and timely.

  • Deliver care in-office, in the community, and in clients’ homes.

  • Participate in team meetings, supervision, and agency events.

  • Support colleagues with case coverage when needed.

  • Transport clients as needed.

 

Who You Are:

​

  • Master’s degree in a mental health field (LCSW, LMFT, LPC or eligible).

  • Experienced or newly licensed clinicians encouraged to apply – we support your growth!

  • Skilled in providing therapeutic support to youth with emotional/behavioral challenges.

  • Passionate about helping families strengthen relationships and build resilience.

  • Motivated to make a meaningful difference in the community.
