At CHR’s Pathways Program, you’ll join a multidisciplinary team that works every day to reduce harm, restore hope, and help individuals reclaim their lives.

Pathways is an accredited opioid treatment program (OTP) specializing in methadone-based MAT. They offer comprehensive residential care—including medication management, counseling, psychoeducation, recovery coaching, and individualized treatment planning—in a deeply supportive and mission-driven setting.

What You’ll Do:
Conduct psychiatric and medical evaluations
Initiate and manage MAT (including methadone and buprenorphine)
Diagnose and treat common acute health conditions (e.g., infections, wounds, respiratory illnesses)
Perform annual physicals and health assessments
Manage mental health medications and assess post-acute withdrawal symptoms
Educate patients on harm reduction, health maintenance, and wellness
Collaborate with your team to develop individualized treatment plans
Guide and oversee RN staff as they carry out medical orders
Maintain accurate and timely clinical documentation
Participate in team meetings, referrals, and coordinated care planning
Who You Are:
Graduate of an accredited PMHNP, APRN, or PA program
Licensed in the State of CT as a PMHNP, APRN or PA with an active DEA license, or ability to obtain
3 years of post-graduate experience working with dually diagnosed adults preferred
Committed to patient-centered, trauma-informed, and recovery-focused care
Comfortable working in a dynamic, team-based environment with a diverse client population
MAT experience strongly preferred (training available for the right candidate)
